For your helper, I need:
1 His name
2 A photo with a neutral background (head shot with no hat or sunglasses)
3 Photo of his Crim Check
4 Photo of his Picture ID

For the business I need:
1) Photo of paperwork with CVOR #
2) Photo of paperwork with Business #
3) Photo of paperwork with WSIB #
4) Registration and ownership for truck (both sides)
5) Certificate of Insurance forwarded to your broker to get them to update with the verbiage in regards to RXO Last Mile
6) SIN#
