S.E.I.U. Local 1 Canada – Paramedical Resident and Family Liaison

Area of Assignment: Long Term Care

Basic Function and Scope:

The Resident and Family Liaison provides social and emotional support to residents and their families to promote adjustment, communication, and well-being within the long-term care home. Working collaboratively with the interdisciplinary team, the Liaison assists residents and families in navigating transitions, accessing services, and fostering a positive and inclusive home environment in alignment with the Fixing Long-Term Care Act, 2021.

Hours of Work: Monday to Friday, days

Status: Part-Time (new vacancy)

Pay Rate: $33.95-$43.27

Education:

  • Diploma or equivalent in Social Services Work or related human services field from a recognized institution.

Experience:

  • Experience with older adults, LTC, community, or health/social sector preferred.
  • Basic knowledge of the Fixing Long-Term Care Act, 2021, the Residents’ Bill of Rights, and LTC admission/discharge processes.

Registration:

  • Eligibility or registration (preferred) with the Ontario College of Social Workers and Social Services Workers (OCSWSSW).

Required Skills:

  • Strong Communication Skills, Interpersonal, and organizational skills.
  • Ability to work collaboratively in a multidisciplinary team.
  • Proficiency in electronic documentation systems (e.g., Point Click Care) and standard office software (Microsoft Office).
