Nurse Practitioner

Area of Assignment: Long Term Care

Basic Function and Scope:

We are seeking a Nurse Practitioner (NP) to join our clinical team. The NP will provide advanced nursing care, clinical expertise, and leadership in the management of residents’ health. Working collaboratively with the multidisciplinary team consisting of Physicians, Nurse Practitioner team, the Director of Care, Registered Nurses, and Personal Support Workers, the NP will support evidence-based care, improve resident outcomes, and strengthen capacity across the care team. In addition to providing care within the Long-Term Care Home, the NP will have opportunities to gain valuable experience as part of the broader LACGH team, supporting Convalescent Care and other hospital departments as needed. This provides a unique opportunity to build skills in both long-term and acute care settings, ensuring continuity of care and professional growth.

Hours of Work: Rotating shifts (days, evenings, nights, and weekends), as required.

Status: Full-Time

Pay Rate: $63.66-$74.46

Qualifications:

Education:

 Current registration in good standing with the College of Nurses of Ontario (CNO) as a Nurse Practitioner (Primary Health Care NP-PHC or Adult NP).

 Nurse Practitioner’s Certification from a recognized program;

 Baccalaureate in Nursing (B.Sc.N.), Masters of Science Degree in Nursing (M.S.N.) preferred.

 Current Certification in B.C.L.S., A.C.L.S.,

Experience:

 Minimum of two years recent clinical nursing experience relevant to chronic disease management, emergency care and/or urgent care.

 Demonstrated excellence in interpersonal and communication skills (written and oral);

 Demonstrated ability to work as a member of an interdisciplinary team.

Required Skills:

 Knowledge and familiarity with computer skills;

 Ability to work independently and collaboratively with members of the health care team;

 Excellent leadership, interpersonal, critical decision making and problem solving skills;

 Contributes to the development and implementation of policies, procedures, and educational programs;

 Ability to analyze and interpret valid research findings in clinical practices;

 Ability to function in crisis situations;

 Ability to work with interruptions/distractions, with constant readjustment of priorities;

 Ability to work in rapidly changing environment/turnaround times;

 Ability to meet mental demands in crisis management/stress management, conflict resolution.

Physical Requirements:

The applicant must be able to meet the physical demands of this position.
